Patient Care Pharmacy uses and discloses protected health information (PHI) only to fill your prescriptions, coordinate your care with your prescribers and insurance plan, and meet legal requirements. We do not sell your health information, and this website does not collect PHI.
Your rights
- Inspect and receive a copy of your pharmacy records.
- Request corrections (amendments) to your records.
- Request restrictions on how we use or share your information.
- Receive an accounting of certain disclosures we have made.
- Request that we contact you in a specific way or at a specific location (confidential communications).
- File a complaint without any effect on your care.
Changes to this notice
We may update this notice from time to time. When we do, the new version will be posted on this page with a new effective date, and a copy will be available at the pharmacy counter.
